FLUOROSCOPY-GUIDED INTERVENTIONAL PAIN PROCEDURES
At CDL Rehabilitation Center, selected interventional pain procedures may be performed with fluoroscopic guidance when clinically indicated. Fluoroscopy provides real-time X-ray imaging that helps the physician identify anatomical structures and guide needle placement during certain procedures.
The type of procedure, spinal level, approach, medication, and use of image guidance are determined by the treating physician based on the patient’s diagnosis, clinical findings, available imaging, prior treatment response, and individual risk considerations. Not every procedure is appropriate for every patient.
Facet-Mediated Pain & Radiofrequency Procedures
Diagnostic Medial Branch Blocks
In selected patients with suspected facet-mediated pain, diagnostic medial branch blocks may be used to help determine whether the targeted medial branch nerves are contributing to the patient’s pain and whether radiofrequency ablation may be appropriate.
Radiofrequency Ablation (RFA)
Radiofrequency ablation is a minimally invasive procedure that uses radiofrequency energy to create a controlled thermal lesion in selected sensory nerve branches involved in transmitting pain signals. RFA may be considered for appropriately selected patients, commonly after clinical evaluation and diagnostic blocks when applicable. Response and duration of benefit vary among patients.
Cervical Radiofrequency Ablation
A fluoroscopy-guided procedure that may be considered in appropriately selected patients with suspected cervical facet-mediated pain after clinical evaluation and, when applicable, a favorable response to diagnostic medial branch blocks.
Thoracic Radiofrequency Ablation
A fluoroscopy-guided procedure that may be considered in selected patients with thoracic facet-mediated pain after appropriate diagnostic evaluation. Clinical indication and target levels are determined by the treating physician.
Lumbar Radiofrequency Ablation
A fluoroscopy-guided procedure that may be considered for appropriately selected patients with lumbar facet-mediated pain after clinical evaluation and, when applicable, diagnostic medial branch blocks.
Epidural Steroid Injections
Cervical Transforaminal Epidural Steroid Injection
A targeted image-guided epidural injection that may be considered in carefully selected patients with cervical radicular symptoms when clinically appropriate. The decision to use a transforaminal approach is made by the treating physician after review of the patient’s clinical findings, imaging, potential benefits, and procedural risks.
Thoracic Transforaminal Epidural Steroid Injection
A targeted fluoroscopy-guided procedure that may be considered in selected patients with clinically appropriate thoracic radicular symptoms. Patient selection, spinal level, approach, and medication are determined by the treating physician.
Lumbar Transforaminal Epidural Steroid Injection
A fluoroscopy-guided epidural injection that may be considered for selected patients with lumbar radicular pain or other appropriate indications. Medication is delivered near the affected spinal nerve region according to the physician’s clinical assessment and imaging findings.
Cervical Epidural Steroid Injection
An image-guided epidural injection that may be considered for selected patients with cervical radicular symptoms or other clinically appropriate indications. The specific approach is determined by the treating physician based on anatomy, imaging, symptoms, and individual risk considerations.
Lumbar Epidural Steroid Injection
An image-guided injection into the lumbar epidural space that may be considered for selected patients with radicular symptoms or other clinically appropriate indications. Treatment selection is based on the patient’s history, examination, imaging, and overall clinical presentation.
Caudal Epidural Steroid Injection
A fluoroscopy-guided approach to the epidural space through the sacral hiatus. It may be considered in selected patients when the treating physician determines that this approach is clinically appropriate.
Treatment Goals
Treatment goals may include reducing pain, improving function, and supporting participation in rehabilitation or daily activities. Individual response and duration of benefit vary.

As with any medical procedure, potential benefits, risks, alternatives, and expected outcomes are reviewed with the patient before treatment. Specific risks vary according to the procedure, spinal level, medical history, medications, and individual clinical factors. The treating physician determines whether an interventional procedure is medically appropriate after reviewing the patient’s clinical presentation and relevant diagnostic information.
